How to Report a Supplement Side Effect to FDA
To report a supplement side effect, stop using the product and get medical care. FDA says to submit a report through the Safety Reporting Portal at safetyreporting.hhs.gov. Keep the bottle, note the lot code and your symptoms, and tell the seller too.

How do I report a supplement side effect?
Stop using the product, then file a report through FDA's Safety Reporting Portal at safetyreporting.hhs.gov (FDA, Report a Supplement Problem).
FDA says if you think a supplement caused a serious reaction or illness, "immediately stop using the product" (FDA, Report a Supplement Problem). FDA also says to stop, seek medical advice and report a bad reaction (FDA 101: Dietary Supplements).
Stop and get care
Call your doctor, or emergency services if severe.
Keep the evidence
Hold on to the bottle, the box and any receipt.
Write it down
Note the product, the date you started and the symptoms.
File the report
Use the Safety Reporting Portal.
What counts as a serious reaction?
FDA lists allergic reactions, heart symptoms, severe gastrointestinal problems, bleeding, jaundice and stroke-like symptoms as examples (FDA, Report a Supplement Problem).
A mild upset stomach may not qualify, but you can still tell your pharmacist and the seller. Reports help FDA spot patterns across many people.
| Example | Action |
|---|---|
| Allergic reaction | Stop, get care, report |
| Heart symptoms | Stop, get care, report |
| Severe stomach problems | Stop, get care, report |
| Bleeding or jaundice | Stop, get care, report |
| Stroke-like symptoms | Emergency care, then report |
What details should I gather first?
Gather the product name, the lot code, the dates, your symptoms and your medicines. A short, clear report is easier to use.
For MemoPryl, the images we have show no lot code, and a seller reference is not lab proof. Read the code printed on your own bottle. See the lot and CoA page for the codes the seller lists.
Keep taking prescribed medicine as directed, and tell your doctor which supplement you stopped.
Add your medicines, your age range and any health conditions. Say how long you used the product before the problem began, and whether the symptoms eased once you stopped. Photos of the label, front and back, are useful if you still have the bottle. Do not throw the bottle away until the report is filed and you are told you no longer need it.

See packsShould I tell the seller as well?
Yes. FDA says supplement firms must report serious adverse events to FDA within 15 days, so telling the company starts that process (FDA 101: Dietary Supplements).

FAQ01
Questions people ask about MemoPryl
Who should hear about a bad reaction to a supplement?
Use FDA's Safety Reporting Portal at safetyreporting.hhs.gov. Stop the product first and get medical advice. Report even if you are not sure the supplement caused it; FDA uses reports to look for patterns.
Do I need a doctor's note to file a report?
No. Anyone can file. A clinician's notes can add detail, but you can report with the product name, the dates and your symptoms. Get medical care first if the reaction is serious.
Do supplement companies have to report problems to FDA?
Firms must report serious adverse events to FDA within 15 days. That is why telling the seller helps, but you can also report directly through the Safety Reporting Portal.
